BENEFITS OFFERED BY TRINITY
**HEALTH INSURANCE:
80% Employer paid and 20% employee paid for employee
Dependent coverage available
**LIFE INSURANCE:
Basic Life & AD&D $10,000 – 100% paid by Trinity. Employee must have health insurance to be eligible.
Optional Life & AD&D – 100% Paid by Employee. Available for employee and family.
**DENTAL INSURANCE:
50% employer paid and 50% employee paid (includes dependent coverage)
**VISION INSURANCE:
100% paid by employee
**LONG TERM:
LONG TERM is 10% paid by employer and 90% paid by employee.
CAFETERIA PLAN:
A cafeteria plan allows employees to pay insurance premiums on a pre-tax basis, reducing their total taxable income and increasing their spendable/take-home income.
**SUPPLEMENTAL INSURANCE PLANS: AFLAC INSURANCE:
100% paid by employee: Accidental, Hospital, Critical Illness/Cancer and Short Term policies available
401(k) SAVINGS PLAN:
Eligibility Requirements: Employee will be eligible to participate in the 401(k) plan after serving 240+ hours within the first 3 months of service. Employees who do not meet the service requirements within the first 3 months of service will become eligible after serving 1,000 hours in the first 12 months of service.
Entry Date: Employee will be able to enter the 401(k) plan on the first day of the quarter on/after satisfying eligibility requirements.
Match: We offer a discretionary match to our employees that is determined at the end of the year. Generally we match up to 3% of employee’s annual salary when the employee contributes at least 6% of their annual salary.
Employee Contribution Vesting: Employee contributions to the plan are 100% vested immediately.
Employer Contribution Vesting: Employer contributions are on a 5 year graded vesting schedule.
CEU PROGRAM:
$750 a year after the first 6 months of employment
PROFESSIONAL LICENSURE REIMBURSEMENT:
100% paid for by Trinity after 6 months of employment
PAID TIME OFF:
Accrual rate: .08333 X hours worked
